Cancellations

We request 48 hours notice of cancellation of an office appointment and 72 hours for cancellation of any procedure. A cancellation fee is applied if you do not notify our office with this notice or fail to keep your scheduled appointment/procedure.

Finances

Co-pay and deductibles are collected at the time of service provided as required by California Law Business Professional 1680U. Installed payments may be considered on a case-by-case basis and must be arranged in advance. We take American Express, Discover, MasterCard and Visa and Cash.

Authorization

We are specialist providers of Physicians Medical Group of San Jose (PMG), Santa Clara County IPA (SCIPA), Premier Care Medical Group, Valley Health Plan, Affinity Medical Group and Northern California Physicians Network (NCPN – formerly known as NCAMG).

We are required to obtain authorizations from primary care physicians prior to an office consultation. Subsequently, we can initiate authorization requests for patients for follow-up appointments.

Office Hours

Office hours are Monday-Friday 9 AM – 5:00 PM. We respect your time and try our best to keep your appointment time within 15 minutes. If you are more than 30 minutes late, we may have to reschedule your appointment.
